While working with a patient, a provider will note any pertinent information about certain aspects of a patient’s social history, such as smoking/tobacco status, alcohol use, birth control use and drug use. The collected information is available in i2b2, sourced from Epic. Legacy social history information is not available in i2b2, but may be available in the CDW-H by request.
